Medicare

All our therapists are registered providers of Medicare Australia. You or your child may be eligible for one or more of the following Medicare plans.

Chronic Condition Management Plan (CCMP), previously known as the Enhanced Primary Care Plan (EPC):

Chronic Conditions Management Plan (CCMP) or a GP Chronic Conditions Management Plan (GPCCMP) is a structured plan of care developed by a General Practitioner (GP) for patients who have a chronic medical condition. Since July 1st 2025 a single CCMP replaced older plans like Chronic Disease Management Plan (CDMP), Team Care Arrangement (TCA) and Enhanced Primary Care Plan (EPC).

Client Eligibility

  • You have a chronic or terminal condition lasting six months or more.

  • Your GP believes you’d benefit from a structed care plan

Which of our Services can use a CCMP plan for?

  • Physiotherapists

  • Occupational Therapists

  • Speech Pathologists

Cost to the Patient

  • Up to 5 allied health visits per calendar year are subsidised by Medicare (rebate varies by profession).

  • Our clients are required to pay for the full service and claim back from Medicare.

 How Do I Get a Plan?

  1. Make an appointment with your GP
    Let the receptionist know you’d like to discuss a "Chronic Conditions Management Plan" or "CCMP".

  2. Work with your GP
    You’ll talk about your condition, set goals, and decide on which services you need.

  3. Get your referrals
    Your GP will give you referral forms to take to the allied health professionals (Sensia Allied Health)

  4. Book your appointments
    Call the providers directly and book in. Take your referral form with you.

Complex Neurodevelopmental Disorder Services:

Clients can be given access to 20 subsidised sessions for either Occupational Therapy, Speech Therapy or Physiotherapy, by a Paediatrician. This Medicare Scheme is for clients who have an Autism Diagnosis and are awaiting funding with NDIS.

National Disability Insurance Scheme (NDIS)

Our clinic is a registered provider under the National Disability Insurance Scheme (NDIS) for agency managed, self managed and plan managed participants. 

Our clinic is ISO certified through SAI Global and accredited with the 5 Ticks of Quality Assurance, demonstrating our commitment to high clinical and professional standards and gives you added confidence in the quality of care you’re receiving.
